Hello, You would need a hole in the door to fit the 8mm spindle through, but no large holes are required. This is fitted like a standard mortice knob with a mortice tubular latch such as: https://www.ironmongerydirect.co.uk/product/altro-heavy-duty-tubular-latch-103mm-case-82mm-backset-florentine-bronze-314530 Thank you
